I can't really help with vB 4.0, don't use it, but you should have an option to enter the button image address when adding the code. As for the CSS, maybe add to a template or style var, I don't know the system here.
Add the css to CSS Templates > vbulletin.css, anywhere in there should work im not sure about the images though.
